Uterine Fibroids and Menopause – should you ‘wait and watch’? Is Uterine Fibroid Embolization recommended?

Uterine fibroids are benign tumors that are usually found in the tissue of the uterus. Uterine Fibroids often present with abnormal uterine bleeding, pelvic fullness, and may have harmful effects on fertility. Female hormones estrogen and progesterone are believed to stimulate their growth. This is ascertained as Uterine fibroids are observed to contain more estrogen and progesterone receptors than normal uterine muscle cells. 

Generally, it is assumed that around the onset of menopause when the level of hormones starts to drop, fibroids tend to shrink or disappear. However, several research-based studies suggest that the fibroids not only persist after menopause, but some may even grow and cause symptoms.

Symptomatic fibroids, regardless of the patient’s age, can create anxiety in women. For someone in their menopausal age, it is in addition to the existing distressing symptoms like hot flashes, sleep disturbances, and recurring infections in urinary tract.

Fibroids present in perimenopause – the transitional period before menopause, or post menopause usually manifest as abnormal uterine bleeding. Since this is a broad symptom in any woman during this stage and may not be specific to fibroids, women and healthcare providers need to be vigilant and a thorough evaluation is required for accurate diagnosis and proper treatment. 

The “Wait and Watch theory during menopause

For several years, it is observed that doctors would suggest patients close to menopause and having fibroids to just “wait and watch” what happens. The fact is that the duration from peri menopause to menopause varies for each patient and in many women it can take years to see a significant reduction in the fibroid size and reduction in symptoms. 

The problem with the ‘wait and watch’ theory is that very few patients with fibroids make it to menopause and would prefer to undergo a hysterectomy to improve their quality of life. Hormonal imbalances associated with hysterectomy would leave them with a range of physical and emotional stress and symptoms. 

It is also observed that several post-menopausal women continue to suffer pain, and other symptoms caused by uterine fibroids.

Abdominal hysterectomy would require hospital stay which can last from one to three days and recovery time is generally four to eight weeks.

Uterine fibroid embolization (UFE)

An alternative approach is to undergo an embolization procedure. Recognized as an effective non- surgical treatment option for fibroids, the procedure has a high success rate. Studies suggest that around 90 % of women undergoing UFE treatment find relief from fibroid related symptoms and have an improved quality of life.

The UFE procedure is less invasive than any surgical procedure. It is performed without leaving noticeable scars behind. Gradually, patients see improvements such as reduced pelvic pressure and pain, bleeding and improved general well-being.

Multiple fibroids can be treated in one sitting as an outpatient procedure. Recurrence of treated fibroids are rare. Patient recovery time is much less, which means lesser time to just be back to normal again!
